1. Applicants are to be from one of three Woodland Bands (Montreal Lake, Peter Ballantyne Cree Nation or Lac La Ronge Indian Band).

2. Applicants must be enrolled in K-12 School.

3. Your logo design must be submitted by May 6, 2019 along with this application form.
